In exchange for participation in the activity of:

Indoor children's play activities including but not limited to: climbing on soft play structures, sliding, role-play in mini town area, arts and crafts activities, sensory play, active physical play, and general recreational play in a supervised indoor play facility for children ages 0-8.

Organized by Busy Bee Play Cafe LLC, of 7655 Mall Parkway, Suite K, Stonecrest, Georgia, 30038, and/or use of the property, facilities, and services of Busy Bee Play Cafe LLC, I agree for myself and (if applicable) for the members of my family, to the following:

PHOTO & VIDEO CONSENT:

Busy Bee Play Cafe regularly captures photos and videos throughout our facility for promotional use on social media, website, and marketing materials. By signing below, you consent to your child's image being used for these purposes.

If you do NOT want your child photographed, please inform staff at check-in and your child will receive a "NO PHOTO" wristband.
